For existing patients, please fill in the form. We will respond your request within 24 hours on weekdays to confirm your appointment. For new patients, please call our office instead. We are open on Tuesday and Friday in the Hayward office and on Wednesday in the Pleasanton Office. On Monday and Thursday, we accept phone calls ONLY. We are not open on weekends.
